Criminal Background Check (OAC
4723.09) |
Submit both BCI (State) and FBI (Federal) background checks. Any
disqualifying offenses under OAC 4723-7 will result in ineligibility for
admission. |

Provide results of a 1-Step TB test or chest x-ray within one (1) year of
admission. Retesting may be required prior to clinical rotations. |
